Description: This recall involves Philips “Lightolier” brand AF4SY decorative glass lenses. This clear glass lens, used to “soften” light with an additional lens layer, is 4 ¾ inches in diameter and 1/8 inch thick. The lenses were manufactured between January 2007 and December 2013. There are no markings on the product. The product number is on the packaging.
Remedy: Consumers should immediately remove the recalled glass lens accessory and contact Philips Lighting at (800) 634-0138, via email at email@example.com for a free replacement unit.
Sold at: Electrical wholesale distributors nationwide from February 2007 through December 2013 for between $25 and $50.
